Membership is available to those who are both Title 19 & Non-Title 19 enrolled in AHCCCS with a qualifying diagnosis. Other payment options are available.

Great News! We are now accepting General Mental Health & Substance Use Referrals (GMHSU) for the following plans.

• Mercy Care 
• Banner University
• Molina Healthcare
• Blue Cross Blue Shield of Arizona
• United Health Community Plan
• Arizona Complete Health

We invite you to set up a tour. Attendees will be given an opportunity to ask questions, tour the facility, and meet staff.

REN is free for most with no out-of-pocket costs!

REN Referral Process

To begin services with REN the following documents are required:

• Demographic Form (Formerly Form 1013).
• Most Recent Part E(Assessment) With BHP Signature or Affidavit. 
• Most Recent Individual Service Plan (ISP) | ISP must be signed by the individual and BHP.

Once the referral packet is received, a member services specialist will contact the applicant to set up an intake at 212 East Osborn Road. Phoenix, AZ 85012.

Every REN Member is first referred by you, our Partners. You are an integral part of the recovery journey and as such, your needs and satisfaction are important to us. We are committed to working directly with our clinical healthcare providers to provide an excellent continuum of care.

Referral Benefits:

• Help demonstrate the outcomes required by the Substance Abuse and Mental Health Services Administration (SAMHSA)
• Provide Individual Service Plans (ISPs) that track Member progress
• Assist in setting goals for the year with Members and note achievements
• Share detailed information with you about Member care
